Once you have booked on to a course please complete the appropriate form below to register your details with us. Please be as accurate and truthful as possible so we can give you the best service we can.

If you are Under 18 please make sure you complete the U18 version with the help of a parent or guardian.

Evaluation Forms

Now that you’ve completed your course we really want to know how well we did and how we can improve so that everybody gets the best service possible. Please fill in the appropriate form below depending upon if you did a course via the STA or another Awarding Organisation.
